CLINICAL CONFERENCE ON "HEMOLYTIC-UREMIC SYNDROME IN CHILDREN"

On December 7, 2023 at the department of propaedeutics of children’s diseases of the State Educational Institution "Avicenna Tajik State Medical University" the staff of the department, together with doctors of the 7th children's somatic corps of the State Institution "Medical Complex Istiqlol", held a clinical conference on the topic: "Hemolytic-uremic syndrome in children". HUS is a condition associated with the blockage of small blood vessels, leading to the destruction of blood cells and disruption of the function of several organs, primarily the kidneys. HUS is usually caused by infections, often with Escherichia coli (E. coli). HUS (Hasser's disease) is clinically characterized by a triad of symptoms: hemolytic anemia, renal failure with oligoanuria, and thrombocytopenia. Most often intestinal infections in children, which are accompanied not only by frequent liquid bloody stools, but also hemorrhagic rash, decreased urine output, hematuria, edema, lead to acute renal failure. To confirm the diagnosis, a general blood test with an indication of the number of platelets, coagulogram, bacteriological stool culture for flora and sensitivity to antibiotics is studied.

The principle of treatment is to control the volume of circulating blood, correction of anemia and electrolyte disorders, normalization of blood pressure.

Prognosis in HUS in children with timely diagnosis and correctly chosen treatment tactics in 85% of cases is marked by recovery, and in 4-10% of cases the disease progresses to chronic kidney disease. In 5-10% of patients with late treatment, the disease is fatal.

Prophylactic measures should be aimed at the use of fermented milk products that have been pasteurized, personal hygiene and timely medical attention in case of acute intestinal infection.

The presentation was prepared by the physician of the I-st nephrology department, the co-chair of the department Rabieva M.S., who detailed the key points of hemolytic-uremic syndrome in children.   Special attention was emphasized on etiopathogenesis, prevalence, clinical manifestations of HUS, diagnostic methods, as well as differential diagnostic criteria with the main methods of treatment of patients. Information on the main and most common complications was given.

At the end of the presentation there was a discussion and a number of questions related to the diagnosis of HUS, to which Rabieva M.S. provided comprehensive answers.
